Job Description

The Pharmacy Technician Med Reconciliation position greets and registers incoming patients in the practice management system, collects/data enters demographic and insurance information with a high degree of accuracy. The position confirms and schedules appointments and verifies insurance. The position receives incoming telephone calls in a prompt, courteous, and professional manner. The Pharmacy Technician Med Reconciliation also accurately collects and enters current patient medications into the health systems's information system for utilization by the practitioners functioning in the Pharmacotherapy Clinic.

Responsibilities

  • Performs all responsibilities/duties required by the pharmacy as defined by the scope of service to ensure that the unique nature of the client is addressed.
  • Exhibits solid knowledge of medications, strengths available, indications. Excellent ability to use drug information database products to obtain medication information needed to perform assigned functions.
  • Conducts patient or caregiver interviews in order to obtain best possible medication histories for patients admitted to the hospital, including prescription, over the counter, herbal, and nutraceutical medications.
  • Communicates with external pharmacies, physicians or other third-party software regarding any patient medications, refill dates, instructions for use, etc. and confirms current medication history as described by patient where appropriate for clarification, completion of medication history or to resolve any discrepancies.
  • Accurately transcribes medication histories into electronic medication health records for review.
  • Develops draft admission medication reconciliation list.
  • Sets priorities in correct order in relation to the needs and circumstances at the time with reference to the patients and clinic practitioners.
  • Exhibits excellent verbal and telephone communication skills with the ability to quickly create and maintain interpersonal trust with patients, family members, outside professionals, to the extent needed to perform job functions.
  • Maintains current knowledge/certification/licensure. Pursues professional growth and development.
  • Greets visitors or patients and directs them as appropriate using the mission, vision and values of Baptist Health Care.
  • Enters all patient demographics into the practice management system and continually maintains patient information in the patient record.
  • Verifies health insurance eligibility and understands copays, co-insurance and deductibles.
  • Accountable for appointments and scheduling patients properly on the days and times specified, as well as reminding patients of appointments. Responsible for duties including but not limited to the following: coordinates and schedules appointments, ensures appointment reminders and online appointment requests are managed timely, and follows up with patients who "no-show" to reschedule appointment.
  • Receives and routes incoming telephone calls in a prompt, courteous, and professional manner.
  • Operates a variety of office equipment effectively.


Qualifications

Work Experience

  • 1 year Retail pharmacy experience Required
  • 1 year Inpatient pharmacy/medication reconciliation experience Preferred


Knowledge, Skills and Abilities

  • Basic computer skills including MS Word and the Internet.
  • Good oral and written communication skills.
  • Understanding of the medication reconciliation process.
  • Ability to document medication history information using available resources.
  • Be able to make recommendations and handle complaints and requests in a professional and calm manner.
  • Ability to prioritize work.

Education

  • High School Diploma or Equivalent Required

Licenses and Certifications

  • Registered Pharmacy Technician State of Florida Upon Hire Required
