Job DescriptionThe RN - Patient Educator is an advanced practitioner with in-depth knowledge and understanding of the complex physiological and psychosocial dimensions of cardiovascular care for the client and family in the community setting. The functional components of the role include that of clinician, educator, and consultant.
Responsibilities- Provides ongoing clinical direction to the health care team as needed to optimize patient/family transitional care. Serves as a Patient advocate to the cardiovascular multidisciplinary team.
- Identifies and meets educational needs for patients, families and/or support systems to maximize understanding of medical diagnosis and to promote wellness.
- Develops and maintains current evidence-based patient transitional care/education materials for patients/families and staff: meets quality and marketing approval.
- Reviews patient medical records to ensure compliance with clinical quality indicators: AMI, HF,LACE Scores, CV SCIP, STS and ACC registries. Assists in the development of hospital processes specific to cardiac nursing.
- Collaborates with clinical nursing teams, physicians and advanced providers, case management, pharmacy and social work to provide holistic patient care and identify resource needs prior to discharge/transitional care.
- Identifies AMI(STEMI/NSTEMI) and HF appropriate patients and arranges for such patients to be followed-up/cared for post-hospitalization.
- Maintains and updates specialty knowledge and skills based upon current cardiovascular nursing and healthcare practices.
QualificationsMinimum Education- Bachelor's Degree Community Health Education, Education, Nursing, Social Work Required or
- Bachelor's Degree Within two (2) years of employment Required
Minimum Work ExperienceLicenses and Certifications- Registered Nurse Licensed State of Florida or eligible compact state Upon Hire Required
- BLS for Healthcare Providers (BLS) American Heart Association Upon Hire Required
- National Institutes Of Health Stroke Scale (NIH) Upon Hire Required
